Registration Materials

The following information is required at the time of registration:
Birth Certificate (copy only)

• Baptismal Certificate (if Catholic) (copy only)
• Social Security Number
• Recent report card for students entering grades 1-8
• $50.00 per child non-refundable registration fee
• Custody papers if parents are separated or divorced
